If you’re looking for a sweet but simple Mother’s Day idea that works well for preschoolers, consider a Mother’s Day card tray with an All About My Mom or Grandma questionnaire. A preschooler can make a special design for the front of the card, and a Mother’s Day questionnaire can be a wonderful (and sometimes hilarious) keepsake.

Simple Mother’s Day Card Tray with Mother’s Day Questionnaire
For the tray, I just used a Multicraft tray, colored cardstock, stickers, art materials (such as Pentel oil pastels) of your choice, and a glue stick for attaching the completed questionnaire to the inside of the card.
For an older child, you could have a simple card-making tray available in a closed cabinet in your child’s room or in a lower kitchen cabinet. It’s nice if the cabinet has a door that closes so your child can feel the creation of cards is a special surprise.
I always had colored construction paper or typing paper, markers, and stickers available for card-making projects. My children liked to make cards for holidays like Mother’s Day, but they also made cards spontaneously at other times.
I also recommend keeping a box of 5¾” by 8¾” greeting-card envelopes available for projects like this. The envelopes can be purchased at any office-supply store and give your child an easy, professional-looking way to finish the card. My children have made their own envelopes before as well, although purchased envelopes can still be personalized with markers, stickers, etc.
Free All About My Mother and All About My Grandma Questionnaire Printables
There are a number of wonderful Mother’s Day questionnaires available online. Here are some sweet ones that are very similar. Just choose one that has the design or colors you want to use.
The Mother’s Day questionnaire for mom or grandma from Crafting Chicks is the one I used with my 3-year-old granddaughter, Zoey, when she made a card for her mom. A child could make any type of design on the front, a flower similar to the one pictured or an open-ended design created by the child. Some of the open-ended Mother’s Day designs my kids created for me were my very favorites. I think the questionnaire is perfect for the inside of the card. It’s such a fun keepsake when a preschooler’s answers are used for the questionnaire.
The “All About My Mom” or “All About My Grandma” Mother’s Day questionnaire from Crafty Morning could include a drawing of mom or grandma at the front or inside of the card.
